Clinical Pharmacy Team

Clinical Pharmacists and Pharmacy Technicians are important members of the multidisciplinary team. 

Our Clinical Pharmacists provide structured medication reviews for our patients, such as those who are frail or on high risk medication prescriptions. Our Pharmacy Technicians manage medication queries and drug reconciliations, and conduct patient reviews.

Pharmacists look at the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with multiple conditions, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.
